Field Experience and Teaching Practice Score Submission

Submit one score-sheet category at a time. Each submission is routed to the selected department and remains Pending Verification until it is reviewed and approved.

Choose the category represented by the Excel score sheet you are submitting. A separate submission is required for each category.

Assessment Type *

Select an assessment type to see the required score columns.

Excel Score Sheet *

Upload the approved score-sheet template for the selected category. Each template is available separately under Resources.

The file must contain S/N, Registration, Name of Student, and the score column(s) for the selected assessment.

Select the assessment type and score sheet to validate it.

Claim Form Optional

Upload a completed claim form if applicable. The Developer/System Administrator can change this field between Optional and Compulsory.

Accepted formats: PDF, Word or Excel.

Electronic claimant declaration
I certify that this claim is accurate and authorize its submission for processing. Confirm the verification email before HoD approval.

Only this Field Experience and Teaching Practice workflow uses these six score-sheet categories. Other submission portals are unchanged.
