Submit your scores for departmental verification. Field Experience V and Micro-Teaching require only score sheets because tutors were paid during face-to-face sessions. Payable activities require a completed claim form.

Excel Score Sheet *

Upload the approved score-sheet template for the selected category. Each template is available separately under Resources.

The file must contain S/N, Registration, Name of Student, and the score column(s) for the selected assessment.

Upload the completed claim form. The number of candidates claimed must match the candidates listed in the attached score sheet.
